Re(h)ability4Life – proyecto de mejora continua de la calidad de Enfermería de Rehabilitación en cirugía cardiotorácica

Introducción: La intolerancia a la actividad (IA) es frecuente en las personas sometidas a cirugía cardiotorácica, comprometiendo la capacidad funcional y la calidad de vida. A pesar de su elevada prevalencia, el diagnóstico de la IA y la implementación de intervenciones dirigidas a este foco continúan estando insuficientemente valorados en la práctica clínica.
Metodología: Proyecto de mejora continua de la calidad, basado en el ciclo Deming-PDCA y en la metodología de Heather Palmer, desarrollado con el objetivo de aumentar la efectividad diagnóstica y la tasa de resolución de la intolerancia a la actividad como indicadores de calidad asistencial. Para ello, se implementó un modelo estructurado de cuidados de Enfermería de Rehabilitación dirigido a personas con IA. Se definieron indicadores de estructura, proceso y resultado. Los datos se obtuvieron a partir de los registros realizados en SClínico® y de los informes de actividad de la unidad entre 2022 y 2023.
Resultados: Se incluyeron 2.634 personas sometidas a cirugía cardiotorácica con IA en el marco del proyecto Re(h)ability4life. Durante el período de implementación se observó un aumento progresivo de la tasa de efectividad diagnóstica de la IA, que alcanzó el 93,89%, así como de la tasa de resolución, que llegó al 85%. El modelo estructurado de cuidados de Enfermería de Rehabilitación fue implementado en el 100% de las personas con diagnóstico de IA, contribuyendo a la estandarización de la práctica clínica.
Conclusión: La IA constituye un foco sensible de los cuidados de Enfermería de Rehabilitación en las personas sometidas a cirugía cardiotorácica. El proyecto Re(h)ability4life contribuyó a mejorar la efectividad diagnóstica, aumentar la tasa de resolución de la IA y sistematizar los cuidados, como lo demuestra la evolución favorable de los indicadores de calidad asistencial monitorizados.
