Re(h)ability4life- projeto de melhoria contínua da qualidade de Enfermagem de Reabilitação em cirurgia cardiotorácica

Introdução: A intolerância à atividade é frequente na pessoa submetida a cirurgia cardiotorácica, comprometendo a capacidade funcional e a qualidade de vida. Apesar da sua elevada prevalência, o diagnóstico e a implementação de intervenções dirigidas a este foco permanecem insuficientemente valorizados na prática clínica.
Metodologia: Projeto de melhoria contínua da qualidade, baseado no ciclo de Deming-PDCA e na metodologia de Heather Palmer, desenvolvido com o objetivo de aumentar a efetividade diagnóstica e a taxa de resolução da intolerância à atividade enquanto indicadores de qualidade assistencial. Para o efeito, foi implementado um modelo estruturado de cuidados de Enfermagem de Reabilitação dirigido à pessoa com IA. Foram definidos indicadores de estrutura, processo e resultado. Os dados foram obtidos através dos registos efetuados no SClínico® e dos relatórios de atividade da unidade, entre 2022 e 2023.
Resultados: Foram incluídas 2634 pessoas submetidas a cirurgia cardiotorácica com IA, no âmbito do projeto Re(h)ability4life. Durante o período de implementação verificou-se um aumento progressivo da taxa de efetividade diagnóstica da IA, que atingiu 93,89%, e da taxa de resolução, que alcançou 85%. O modelo estruturado de cuidados de Enfermagem de Reabilitação foi implementado em 100% das pessoas com diagnóstico de IA, contribuindo para a uniformização da prática clínica.
Conclusão: A IA constitui um foco sensível dos cuidados de Enfermagem de Reabilitação na pessoa submetida a cirurgia cardiotorácica. O projeto Re(h)ability4life contribuiu para a melhoria da efetividade diagnóstica, da taxa de resolução da IA e para a sistematização dos cuidados, evidenciada pela evolução favorável dos indicadores de qualidade assistencial monitorizados.
